The skill read like an authoritative checklist; clearing its items could be
mistaken for a complete review. Reframe it explicitly as guidance — a
where-to-look map that lowers startup cost, not a definition of a sufficient
review. Add a "How to think about a review" section drawing on the patterns
from the /code-review, /review, requesting-code-review, and
receiving-code-review skills: reason from the code independently, sweep all
aspects (correctness, concurrency, security, design/approach, tests, docs,
…), verify before flagging, calibrate confidence and suppress noise, and lead
with severity. Add a receiving-review note so authors evaluate findings on
merit rather than following them blindly.
Codex review of PR1 found a semantically-identical stale claim outside the
three files first touched: .agents/skills/dsh-code-review/SKILL.md said "the
doc-sync rule has no gate", which is the same P1-16 drift. doc-sync DOES gate
compilable ts blocks and the event-taxonomy table; only prose drift (config
keys, defaults, error codes, wire fields) is ungated. Reworded to say exactly
that.
(ADR 0014 was also checked and is correct as-is — it is the decision record
that establishes the gate and already describes it as existing.)
- AGENTS.md Commands: fix typecheck/build descriptions; add lint, lint:fix,
test:coverage, knip, publint, hygiene (were undocumented).
- Drop the bare `yarn demo` for explicit `demo:echo` + `demo:coding`; update
README, examples READMEs (and document coding-agent in examples/README).
- New cookbook guide: adding-a-vendored-package.md (the missing "add" half of
vendor/README's update-only procedure).
- architecture.md: add a table-of-contents and extract the Extension cookbook
to docs/cookbook/extension-cookbook.md (link-preserving); drop the completed
"restructure this document" TODO.
- ADR 0009 (capability seams) + 0010 (twin LLM adapters), and a "when to write
an ADR" standard in adr/README.
- Add a committed dsh-code-review skill under .agents/skills, exposed to Claude
Code via a tracked .claude/skills symlink (gitignore carve-out).
